Access to Care Program

The Acess to Care program is a unique, non-profit primary health care program for low-income, uninsured residents of suburban Cook County and northwest Chicago.  Undocumented residents are eligible to apply for the program as well as legal residents living here for less than 5 years.

The program provides doctor office visits for $5, some lab and X-rays for $5 and prescription medications for $15 (generic), $30 (preferred brand) and $40 (non-preferred brand).  There is a non-refundable annual enrollment fee of $20 per person, $40 for two people, or $50 for three or more.

To be eligible for this program you must:

  • have family income of less than 300% of the federal poverty guidelines

  • not have health insurance (or deductible of $500 or more per person)

  • be ineligible for Medicare, Medicaid, CountyCare, All Kids and Family Care
