2017 Apple Tree Enrollment Form
Grades K-9th grade only
If you would like to participate in the 2017 Palatine Township Apple Tree Campaign please complete the form below.
- Enrollment Forms must be submitted to Palatine Township no later than June 12, 2017.
- Please select pick up times at the bottom of this form. (1st choice, 2nd choice and 3rd choice.)
AN APPLICATION SHOULD BE COMPLETED FOR EACH CHILD INDIVIDUALLY.

*If accepted, you will be contacted by email by Friday, June 30, 2017, with information for required for the program.
If you do not receive an email from the Township by June 30, call the Township at 847-358-6700 for notification.
Backpacks and school supplies will be distributed at Palatine Township:
Thursday, August 3, 9:00 AM-6:30 PM,
Friday, August 4, 9:00 AM-3:30 PM, and
Saturday, August 5, 9:00AM-11:30 AM.
All participating children must be present in order to receive supplies.
If they are not able to attend, please do not enroll.
